Athens Eats! Recipe Submission Form & Permission to Publish Agreement

This form is for the submission of a recipe for inclusion in the book tentatively titled Athens Eats (hereinafter referred to as the “Book”) to be published by AIDS Athens (hereinafter referred to as the Publisher”). Please use a separate form for each recipe. Please complete the form fully and click "Submit" when finished.. Receipt of your recipe will be acknowledged.

By submitting this Recipe Submission Form and Permission to Publish Agreement, I hereby grant to AIDS Athens, as Publisher of the Book, non-exclusive permission to use my name and recipe(s) in and on any and all editions of the Book as well as in any promotional materials to encourage publicity for sales of the Book. I am giving one-time permission to the Publisher to publish my recipe and I continue to own my recipe and may use it in any manner as I see fit. Information provided on this form may be used and may also be copyedited for routine and standard style and use considerations. I understand that the copyright for the Book shall be registered to the Publisher and that I will receive no compensation for the contribution of my recipe. I understand that the Publisher will provide me with one complimentary copy of the Book upon publication, sent to me at the address provided herein. I understand and acknowledge that the Publisher is not obligated to include my recipe in the Book or subsequent editions of the Book. I warrant that I have full power to provide this recipe to the Publisher and that this does not conflict in any way with any other commitment I have to any other party.
